Pursuant to the provisions of California Health and Safety Code § 11362.775, this agreement is entered into between the undersigned and C.H.S. Unlimited, a nonprofit mutual benefit Collective operating a medical marijuana collective pursuant to California law (referred to below as "Collective").   The undersigned certifies the following under penalty of perjury under the laws of the State of California:

  1. I have the right to obtain and use marijuana for medical purposes pursuant to recommendation of a California I am a qualified medical marijuana patient who is entitled to the protections of California Health and Safety Code Sections 11362.5, and 11262.7 et seq.
  1. A true and correct copy of my current written physician's recommendation or approval for the medical use of marijuana is attached hereto ("Recommendation"). At such time that it may expire I will provide a current copy to the
  1. By my signature below constitutes my written release per Health Insurance Portability and Accountability Act (HIPAA) authorizing the Collective (i) to confirm my Recommendation, (ii) to permit members authorized to grow, process and transport medical marijuana on the Collective's behalf, to possess and present to law enforcement my current Recommendation as needed and (iii) to keep me informed of my Recommendation
  1. I intend to associate with the members of the medical marijuana association, in part, through this agreement, in order associate for the purpose of ensuring access to medical purposes pursuant to the Medical Marijuana Program Act, which includes in part, California Health and Safety Code 11362.775 and Section 1 (b)(3) of the un-codified portion of the Medical Marijuana Program Act.
  1. I understand and agree that I will be sharing marijuana with other Collective members and that I may be charged for the cost to reimburse other members for their labor, supplies, services or materials or transportation costs towards the cultivation, processing or procurement of marijuana for the
  1. That marijuana obtained through this Collective SHALL NOT be shared, given or otherwise distributed non-members and that breach of this covenant shall be grounds for immediate membership
  1. I understand that Collective membership provides no voting or other management rights with respect to the Collective or its operation, and that I am a patient-member
I am 18 years of age or older. I understand the medicinal use of cannabis remains illegal under federal law and circumstances may require defense in state or federal court, which requires the disclosure of patient-members names and medical information, as well as, I may be asked to participate in such defense.




Pursuant to California Health and Safety Code§ 11362.775   We, the members and patient members of the above identified collective (the "Collective"), hereby certify that we are qualified patients suffering from serious medical conditions and have obtained recommendations or approvals from licensed physicians in the State of California to use medical cannabis (marijuana) to treat our medical conditions. This agreement shall be in effect as of May 1, 2015.   COPIES OF OUR INDIVIDUAL MEMBERSHIP AGREEMENTS AND MEDICAL RECOMMENDATIONS ARE INCLUDED AND INCORPORATED HEREIN.   As qualified medical marijuana patients under California law, we choose to associate collectively to cultivate, transport and share marijuana for our medical purposes. All members of our medical marijuana collective will contribute labor, funds or materials and in turn all will receive medicine. We form this collective in accordance with California Health and Safety Code§ 11362.775, which provides as follows:

"Qualified patients, persons with valid identification cards, and the designated primary caregivers of qualified patients and persons with identification cards, who associate within the State of California in order collectively or cooperatively to cultivate marijuana for medical purposes, shall not solely on the basis of that fact be subject to state criminal sanctions under Section 11357, 11358, 11359, 11360, 11366, 11366.5, or 11570."
Unless the local county has guidelines concerning medical marijuana, California state law exclusively governs and permits one (1) patient to cultivate at least 6 mature plants & 12 immature plants and possess as much as 8 oz. (0.5 lbs.) of processed medicine (marijuana flowers and trim).
